2009年9月27日 · Don't forget pertinent negatives when you do PCRs. I've always been told that if you didn't document it you didn't look for it. Example: pt no c/o loss of consciousness, pt no c/o altered mental status, no c/o pain, no c/o SOB, no obvious trauma, no bleeding, no change in physical condition en route, no change in mental status en route, etc. etc.

Every narrative should be different. If you are truly documenting all of your assessments, treatment and events there is no template, just a format.

I very rarely repeat their history in the narrative unless it is SPECIFIC to the run as I've already written that info on another page of the run form.

Document that the patient is AOx4, GCS 15, able to make decisions for themselves, that they were informed and that the understand the risks of refusing treatment/transport, that they are more than welcome to call 911 back at anytime or go to an urgent care/ED, and that by signing the form it releases our company/providers/hospitals from liability.
